A full-time Investigator / Case Worker position is currently available with Liberty Healthcare and the Pennsylvania Department of Human Services Adult Protective Services Program.
Your typical workday in this role will involve traveling to residences, nursing homes and long-term care facilities throughout the region - and conducting investigations of allegations of abuse, neglect, abandonment and/or exploitation of adults who have disabilities.
You will document your findings, develop care plans, and manage approximately 20 active cases.
This position comes with a high degree of autonomy and a flexible work schedule which includes occasional evening hours.
This position will primarily serve (but is not limited to) Elk County & Jefferson County.
Since this position requires regional travel, you must have your own vehicle.
Work-related mileage and toll expenses are reimbursed.
You can expect to be paid approximately $46,000 annually (paid $22.
12 per hour).
Opportunities are also typically available to earn additional income by working optional overtime and taking on additional cases.
You will accrue thirty days of paid time off annually which you may use for vacation, holidays, sick or personal leave.

You are a good fit for this position if you have experience in one of these areas:
Field-based health or human services Service Coordinator or Supports Coordinator Child, Family and Youth Services case management experience (County CYF) Law enforcement / criminal justice Case management or protective services Intellectual and developmental disabilities or behavioral health professional A Bachelor's degree is preferred.
Relevant experience or training will be considered as a substitute for a Bachelor's degree.
Knowledge of Social Assistance Management Systems (SAMS) and Home and Community Service Information Systems (HCSIS) is helpful - and Spanish speaking skills are a plus.
